We listen, encourage and support Service user’s involvement within the action plan stage as much as possible, supporting service users to exercise their rights, choices, and inclusion within society. We Identify short-term and long-term plans, assessing needs, positive risks, goals, and aspirations. We work closely together with service users’ families or other network of support including other health and social care professionals to provide the best quality of support, as well as building up trusting relationships that are often be a lifeline in service users lives.

We cover mental health, learning disabilities, dementia, dual diagnosis, autism, probation services, companionship, and older people.
